1. I hereby agree to send me a distance or off-premise contract’s document or confirmation of contract’s conclusion. I agree to obtain a confirmation of delivering my approval on gaining of digital content in the circumstances resulting in the loss of the right of withdrawal from a contract through an e-mail [consumer’s e-mail address].
2. Pursuant to Art. 38 of the Act on Consumer Rights, conclusion a distance or off-premise contract on providing digital content not recorded on the durable media, approval of the above mentioned providing digital content before the expiration of the right of withdrawal from a contract results in the loss of the right of withdrawal from a contract.
I declare that they informed me about the consequences of giving consent to obtain digital content not recorded on a the durable media before the date of an exercise consumer’s right of withdrawal from a contract.
